Louisiana Community Care

Louisiana Community Care operates five community homes in Ball, Louisiana, providing 24-hour care and services for people with developmental disabilities. Participants of this program select their own learning choices and personal goals leading to independent community living.

The New Opportunities Waiver (NOW) program provides services in the participant's home and is intended to provide specific activity-focused services.  May include up to 24 hour care.

Services through this program are offered for individuals who are elderly or have disabilities by providing services and supports that may not otherwise be available in order for the recipient to remain in their home and community.

The Long Term - Personal Care Services Program provides assistance with daily living activities such as meal planning and preparation, light housekeeping, bathing, dressing, mobility, transportation to medical appointments or shopping, and similar personal care activities.

Provides services to elderly individuals or persons with disabilities in their home and community setting.  Personal assistance services are offered including meal preparation, bathing, dressing, and other self-care activities.  Services can be provided for up to three waiver participants who live together.  Other services offered include nursing services, home delivered meals, housing transition and stabilization services, and assistive devices and accessibility adaptations.

Provides options and meaningful opportunities through vocational and community inclusion for individuals who are 18 years of age or older and meet eligibility criteria for waiver services.

The Adult Day Habilitation Program operates Monday through Friday offering therapeutic habilitation services through vocational  training and job placement in a variety of settings.

The SIL Program offers 24-hour supervision within a person’s own home. The program is designed to be an alternative to nursing home placement or institutionalization. Services include transportation, vocational training/job placement, & recreational activities.  Instruction & assistance is provided for budgeting, meal planning &  preparation, and self-administering medication.
